To join Special Olympics as a volunteer, you need to print and fill out  the Class A Volunteer  Form.  You may return it to any LISD Special Olympics Team coach or Coach Domer.

To join Special Olympics as an athlete, you will need to complete the  Athlete Medical Form.  Please print and fill out this form and return to your coach or Coach Domer.

In addition to the Class A Volunteer  form, LISD Special Olympics Team volunteers must complete the on-line Volunteer Application for the Lewisville School District.

If you are a minor, and want to join Special Olympics as a volunteer, you need to print and fill out  the Minor Volunteer Reference Form.  You may return it to any LISD Special Olympics Team coach or Coach Domer.

If your athlete is a student, you must print and fill out this LISD Student School Related Absence Form in order to participate in activities occuring on regular school days.  Students need to return this completed form to their school office to be excused from school for the days of the Special Olympics events.
